Admissions Process

We ask families and applicants to either Dragonfly TRANSITIONS or Dragonfly ADVENTURES to complete the following steps:

1. Call the admissions office for an initial telephone meeting.
2. Download the application materials:

3. Complete and submit the application packet including contact information and signed Release of Information form for any mental health professionals or treatment programs that can provide further information. If you have discharge summaries, psychiatric evaluations and educational testing and transcripts, please fax or mail copies to us.
4. An on-site or phone interview is scheduled between by the Admissions Director and the applicant.
5. TRANSITIONS applicants and their family members are encouraged to visit Klamath Falls to tour the program, meet the residents and staff, and explore our community.
6. Applicants accepted for enrollment will receive a Preparing for Enrollment packet with additional paperwork that must be completed prior to enrollment and instructions regarding minimum clothing requirements, what to bring (and not bring), and items to consider in making travel plans.
